Topical Caffeine

A topically-applied DHT inhibitor that penetrates the follicle without systemic absorption — the most accessible evidence-backed anti-hair-loss active.

HOW IT WORKS

Topical caffeine directly inhibits 5-alpha reductase activity at the follicle level, reducing local DHT production without systemic hormonal effects. DHT (dihydrotestosterone) is the primary molecular driver of androgenetic alopecia — it binds to androgen receptors in genetically susceptible dermal papilla cells, progressively shortening the anagen phase and miniaturising the follicle over successive hair cycles.

THE SCIENCE

Androgenetic alopecia (pattern hair loss) affects approximately 50% of men and 25% of women by age 50. The molecular mechanism is well-established: testosterone is converted by the enzyme 5-alpha reductase into dihydrotestosterone (DHT), which binds to androgen receptors in the dermal papilla cells of genetically susceptible follicles. Each successive hair cycle becomes shorter — the anagen phase contracts from years to months — until the follicle produces only vellus (fine, colourless) hairs and eventually becomes dormant.

Caffeine was identified as a topical DHT inhibitor through research at the University of Jena (Fischer TW et al., 2007), which demonstrated that caffeine at concentrations of 0.001–0.005% inhibited the testosterone-induced suppression of hair follicle growth in vitro. Crucially, topical application delivers caffeine directly to the follicle without the pharmacokinetic route of oral consumption — meaning follicle-level concentrations are achievable without systemic effects.

A key finding from the Jena study: to achieve equivalent scalp caffeine concentrations via coffee consumption, one would need to drink approximately 60 cups per day. Topical delivery bypasses this limitation entirely.

The clinical timeline for topical caffeine is 3–6 months of consistent use, which aligns with the hair growth cycle — a single anagen-to-telogen-to-telogen cycle. Visible density improvement in clinical studies is typically documented at the 3-month mark.
